INSTRUCTIONS

1. The survey Number/Numbers and name of the village shall be mentioned in capital. 

2. The applicant should clearly mention the complete postal address. 

3.The applicant should submit the application form separately for each village. 

4.The applicant need not attend the office for collection of information as the same will be sent by post. 

5.In case of Municipal limits the applicant should submit a clear site plan. 

Charges for the issue of letter on "land use information" 

1) Rs 50/- per survey number (for outside MCH area) 
2) Rs 100/- per premises number (for MCH area)
